Eligibility
Students must be HOSA members, which means joining through a school chapter; a chapter needs at least five students, a qualified advisor and school administration support. In California the advisor first completes the Cal-HOSA Affiliation Survey and receives state advisor approval, then completes national affiliation and payment at hosa.org. Competitive events are divided into Middle School, Secondary and Postsecondary/Collegiate divisions, and individual events carry their own grade, coursework and certification requirements set out in that year's Competitive Event Guidelines.
Format
HOSA organizes six categories of competitive events: Health Science events such as Medical Terminology, Pharmacology, Pathophysiology and Nutrition, judged mainly by written test; Health Professions events such as Clinical Nursing, Medical Assisting, Phlebotomy and Surgical Technologist, which combine a written test with a scored hands-on clinical skill; Emergency Preparedness events such as CPR/First Aid, EMT and Public Health; Leadership events such as Prepared Speaking, Job Seeking Skills and Organizational Leadership; Teamwork events such as Biomedical Debate, Creative Problem Solving and Health Career Display; and Recognition events such as the Barbara James Service Award and the blood drive program. The progression runs from the school chapter through California's online CE Qualifier, to the Cal-HOSA State Leadership Conference, and then to the International Leadership Conference. Most events use a two-part structure at state and ILC level: a written round that determines finalists, followed by a live skill or presentation round.

Fees
National affiliation dues are $20 per student member and $25 per advisor, paid to HOSA headquarters by the chapter and non-refundable once submitted. Conference registration, lodging and travel for the California State Leadership Conference (SLC) and the International Leadership Conference (ILC) are separate and are announced officially each year; as of September 2026 the registration fee amounts for the 2027 SLC and ILC had not yet been posted on the official site.
Awards
Medals and certificates are awarded at each level, with finalists recognized on stage at the state and international conferences. The Cal-HOSA State Leadership Conference also installs state officers and presents a set of scholarships, including the Board of Directors' Leadership Scholarship, the Merit Scholarship, the Founders Scholarship and several memorial awards. Winners at the state conference earn the right to represent California at the International Leadership Conference, where awards are presented on the final day.

The pathway is clear: students join through a school chapter, California competitors go through Cal-HOSA’s online CE Qualifier, and qualifiers advance to the Cal-HOSA State Leadership Conference, held 1–3 April 2027 in Sacramento. State winners then represent California at the International Leadership Conference, held 22–25 June 2027 at the Pennsylvania Convention Center in Philadelphia, with awards on the final day.

HOSA publishes new event guidelines each summer and adds or retires events, so always check the current year’s guidelines before choosing.

How to prepare

  • First check whether the school already has a HOSA chapter; if not, you need five students, a qualified advisor and administrative approval, which often takes longer than the preparation itself — settle it before September.
  • Choose events by working backwards from the goal: pre-med students should favor Health Professions events such as Clinical Nursing or Phlebotomy, or Health Science events such as Medical Terminology and Pathophysiology, since that material carries straight into AP Biology and later premedical coursework.
  • For written events, study strictly from the reference list and outline in the current Competitive Event Guidelines; the question pool is drawn from those sources, so working the official texts beats general online courses.
  • For skills events, rehearse against the official rubric line by line and record yourself — finals are usually decided by procedural details like hand hygiene, patient identification and waste disposal rather than the core technique.
  • Put the CE Qualifier dates on the calendar: California allocates state conference slots through an online qualifying round, and missing it ends the season, so track the Cal-HOSA important dates calendar.

Frequently Asked Questions

Can my child join HOSA if the Irvine high school has no chapter?

Not individually. HOSA membership runs through a chartered chapter at the student's own school, so if there isn't one, the first step is to get a chapter started: at minimum five students, one qualified advisor, and school administration support. The section above lists which Irvine Unified School District (IUSD) high schools already have a HOSA chapter; if your child's school isn't one of them, it helps to talk to an existing chapter first and bring that model back to your own school.

How do California students join HOSA and which website is official?

California's official affiliate is Cal-HOSA at cal-hosa.org — be careful not to land on a similarly spelled domain with no real content. The process has two steps: the chapter advisor first completes the Cal-HOSA Affiliation Survey and gets approval from the state advisor, then completes national membership affiliation and dues payment at hosa.org. A student is only a full member, and only eligible to enter competitive events, once both steps are done.

Do you have to be in a CTE health science course to join HOSA?

In California, essentially yes. Cal-HOSA treats HOSA as an intracurricular component of the Health Science and Medical Technology CTE pathway rather than as a club: the school needs a qualifying CTE course sequence running, the advisor is normally expected to hold the HSMT CTE credential and teach in that pathway, and student members are generally expected to be enrolled in the pathway. CTE medical course offerings vary across Irvine Unified School District high schools, so confirm the current requirement with your school's advisor and the year's Affiliation Survey.

What does a year of HOSA cost an Irvine student?

Membership itself is cheap: national affiliation dues are $20 per student and $25 per advisor, paid by the chapter to HOSA headquarters and non-refundable once submitted. The real expense is conferences. Families cover registration, flights and lodging for the state conference (Sacramento in 2027) and the International Leadership Conference (Philadelphia in 2027). How much do HOSA awards help with pre-med or college applications?

Placing at the state or international conference is a legitimate award to list in the Common App honors section, and it reads as directly relevant for nursing, public health, pre-med intentions and University of California applications. But HOSA has a very large membership, so simply being a member carries almost no weight. What persuades readers is two or three years of sustained involvement, a specific placement in a specific event, and a chapter office with real activities behind it.
